Josef Silny Course-by-Course Evaluation For Nurses

Step 1: Download the application form at the end of this post.

Step 2: Complete the application form. You can either complete the forms using adobe edit, Foxit reader editor or by printing it and completing it by hand. I will advice when picking a Nursing Board where the report will be sent, you should pick Texas. This is because Texas Nursing Board has unlimited NCLEX attempt, does not require social security and no English requirement is needed. You can change it to any Nursing Board with ease after passing your NCLEX exam

Step 3: After completing the Josef Silny application form, simply send it to the email address on the respective form. You should send the following forms to:

-Biodata form and Consent to release information form: Email both completed and signed forms to info@jsilny.com

-Completed Payment form with a picture of the front and back of your credit card and your international or foreign passport to payment@jsilny.com

-Transcript form: To be sent to your Nursing School who will process it and send it directly to Josef Silny via post.

-License Verification form: To be sent to the Nursing Board in your Home country who will complete it and send it to Josef Silny via Post.

Step 4: Once Josef Silny receives your initial application and completed payment application, it will be processed and the application fees deducted from your account.

Step 5: Within 48 hours after your credit card has been charged, an online account will be created for you and a username and password will be sent to your email. You can then login into your account and monitor the progress of your application.

Step 6: After Josef Silny receives all your documents from your school and Nursing board, it will issue your report within 10-15 days, but recently this has been extending to 20-30days. A copy of your report will be electronically sent to the Nursing Board you indicated on your application form.

8. Can I speak to my evaluator? Yes, if your evaluator needs to speak with you, you will be contacted through your email. However, if you need to book an appointment with your evaluator, you can request that and it cost $60 for 30minutes.
