Mileu Therapy: How Environment Affects the Life of Patients and Caregivers

Is scientific restructuring of environment in order to effect behavioral changes and improve the psychological health & functioning of an individual (Skinner, 1979)

Components of therapeutic environment are:

1)privacy

2)safety & protection

3) comfort.

For the environment to be therapeutic must have these above listed 3 components, absence of any one can affect the therapy of a pt and care giver..that is why in most case TRANSFERENCE & COUNTER- TRANSFERENCE come to exist b/w pt and care giver in which emotional feelings is being transfer between patients and his care giver! Indeed environment is very vital in pt management: Florence Nightingale’s philosophy and techniques emphasized that Healing environment assist pt in recovery!

The concept of therapeutic environment originated in 1959 with Maxwell Jones . He used the therapeutic environment as a treatment modalities that create a correction life experience & nature the healthy aspect of personality that : Manipulation of living conditions including physical facilities, types of therapy & interpersonal relationship would be effective in fostering healthy changes!

Among PRINCIPLES OF THERAPEUTIC MILIEU/ENVIRONMENT are :-

a) basic physiological needs are fulfilled

b)ppl need living &working situation that allow them to feel safe from outside instruction

c) physical facilities are conducive to achievement of goals. And

e) Democratic form of self government exist!

Sincerely speaking the situation we are now in this very institution is not therapeutic, the principal of therapeutic environment is no longer in existing, if the environment become therapeutic and attractive to the staff the organizational goal could be achieved no matter the situation. People left their towns/States and their families to the environment where they think they will spent most of their lives but all in vain… Those responsible should fear Allah and make our environment therapeutic and attractive! For a employee to feel at home and proud of what he is doing environment must be conducive , if not the organizational goal may not be achieved!

I M calling with a strong voice as a Mental healthy/psychiatric nurse that people should fear God avoid using their temporary opportunity and making our environment non-therapeutic, & do anything possible that may promote the wall fare of the employees so that the therapeutic environment and organizational goals could be achieved!

When we are today we are not tomorrow! We will all account for our action on the day of judgment! Long live FTH KT , long live federal republic of Nigeria!

2023: Don’t vote any politician involved in medical tourism – Nigerian Nurses to electorates

Ahead of the 2023 general elections, Nurses, under the auspices of the University Graduates of Nursing Science Association (UGONSA), have admonished Nigerians on whom not to vote.

This formed part of the communique issued by the association at its conference in Abakaliki, on Thursday.

The communique made available to DAILY POST was signed by the UGONSA National President, Chief (Hon.) S.E.O. Egwuenu, and the National Secretary, Nurse G.I. Nshi.

The association said it was beyond shame that the Nigerian health system that once ranked among the best ten in global ranking and benefited from medical tourism treating the likes of the Saudi Royals in the 1960s, was now celebrating that its ranking improved from 187 out of 191 countries.

“How do we come to terms with the Federal Ministry of health’s estimation that Nigeria loses about $2 billion (1.1 trillion naira) annually to Medical Tourism? It is heart wrecking, especially when it is obvious that this money, if put together over the next five years and deployed for the upgrade of our health facilities, health research capabilities, and healthcare infrastructures, can launch our health system to among the best five in the world.

“Rather than pay attention to fixing our health system, our politicians are shamelessly busy competing on whom to win the ignoble title of ‘Medical Tourist 1 of Nigeria.’

“Our hospitals are in shambles because our politicians and their families are not treated in our hospitals. Our hospitals will be fixed to be in the world-class ranking any day our political class starts getting treatment from our hospitals.

“It is high time medical tourism became a topic for the acceptance or rejection of politicians at the polls. Politicians who seek treatment abroad are directly telling other Nigerians that it is their own lives alone that matter and that the rest of us are sub-humans that deserve to be treated in the ‘mere consulting clinics’ they have egregiously reduced our hospitals to.

“The callousness and insensitivity of our politicians to our health system have forced seasoned healthcare professionals, especially doctors and nurses, to seek shelter in the responsive health system of other countries with responsible leaders where their skills, professional development, and remuneration are enhanced. Ironically, our politicians are treated abroad by these same seasoned Nigerian healthcare professionals that left our health system abroad out of sheer frustration.

“The poor performance of our health system in global ranking has never been the fault of our healthcare professionals. It is a systematic fault of not putting the right facilities and motivation in place by the country’s successive leadership to enable our healthcare professionals to replicate the type of wonders they have been performing when they travel abroad in our own health system.

“The monies wasted on medical tourism are enough to rejig our health system and make a competing payment to the healthcare professionals that will dissuade those currently practicing in the country from contemplating going abroad and compel those that have left for greener pastures to return home.

“It is an affront to our collective sensibility to see our politicians abandon our hospitals that they have run aground through age-long fund deprivation and neglect for the treatment of helpless Nigerians that elected them, while they themselves jet out abroad at the slight of a headache to be treated in well organized, well-furnished, and well-funded hospitals with taxpayers’ monies. Such is abominable and unacceptable and must not be allowed to continue,” UGONSA lamented.

The group charged Nigerians “to get their permanent voter’s cards (PVC), prepare to vote out, and never vote for any politician who has a penchant for embarking on medical tourism abroad.

“Politicians whose pastime is medical tourism have testified that they do not believe in the Nigerian system. If our health system is not good for you, our political system should also not be good for you.

“The level of corruption among our politicians is very pathetic as it is obvious that we now practice ‘Lootocracy’ instead of ‘Democracy.’

“The spate of borrowing for consumption and re-looting with no obvious commensurate project on ground is unacceptable to nurses as it is to the generality of Nigerians.

“We, therefore. strongly call for accountability and transparency in governance to rescue the country from its economic doldrums occasioned by financial profligacy as this will have an unimaginable economic enslavement of our unborn future generation.

“The spate of insecurity in this country is also a severe concern to nurses as it has callously led to loss of precious lives that nurses strive to protect and preserve.

“We call on both States and Federal Government to do more in the area of security to tame the ugly trend of worsening insecurity.”

UGONSA also thanked the Head of Civil Service of the Federation for releasing the circular for the proper placement of graduate nurses that was approved by the National Council on Establishment (NCE) at its 43rd meeting held in Abuja in January 2022 and called for its speedy implementation in all health institutions where nurses work.

It, however, said, “Any delay in implementation of the circular will amount to further traumatization of the already traumatized and frustrated nurses who have been psychologically and financially bruised by the inexplicable injustice of under placement meted on them in the civil service over the years despite their unalloyed loyalty to Nigeria and to the patient.”

Ogun State School of Nursing 2022/20223 Exam Result

Ogun Government has announced the release of entrance examination results into its College of Nursing Science for the 2022/2023 academic session.

Commissioner for Health, Dr Tomi Coker, in a statement issued on Friday in Abeokuta, said that applicants could only access their results by logging into the school’s official portal: www.mohsonmportal.com

Coker put the cut-off point for indigenes at 28 out of 60 points and 29 out of 60 points for non-indigenes, saying that information contrary to this should be disregarded.

She added that successful candidates would be contacted via SMS and e-mail for oral interview, scheduled for July 14 at 8 a.m. at the institution’s Abeokuta campus, within the premises of Federal Medical Centre, Idi-Aba.

The commissioner appreciated the candidates who applied for admission into the college.

She said that the Gov. Dapo Abiodun-led administration would continue to be fair, equitable and transparent to all.

First Batch of Kenyan Nurses Move to UK

Thirteen Kenyan nurses have got an opportunity to work in the United Kingdom.

The medics are beneficiaries of a bilateral agreement signed by Kenya and the UK government.

Last year, Labour Cabinet Secretary Simon Chelugui, said it would send 20,000 nurses to the UK as it sought to improve the welfare of its migrant workers overseas.

Initially, the recruitment exercise was marred with controversy when only 10 of 300 nurses passed English language tests required for the NHS jobs.

And yesterday, the government said the 13 nurses had passed the tests and were ready to work in the UK.

Speaking during a ceremony to see them off, Health Cabinet Secretary Mutahi Kagwe said Italy and Kuwait have also expressed their interests to have nurses sent to their countries.

“We have also received an offer for Kuwait but we want to be done with the UK process first. The fact that Kenya’s ambition is to become a health care hub can only happen if the recipients are open-minded about the opportunities,” said Kagwe.

The CS noted that the ministry is building the health workforce with a focus on health promotion which will enable the nurses to come back as entrepreneurs to create more job opportunities locally.

He assured Kenyans that the cohorts have been trained and prepared well so that they can achieve the scores they need.

“The nurses we are flagging off today are well trained and have passed the test of English proficiency. All the candidates have the language ability to be productive in their destination country,” he said. The UK High Commissioner to Kenya, Jane Marriott, disclosed that nurses were recruited by the Oxford University Hospital Trust with an addition of six more nurses to leave the country in about two weeks.

The diplomat was upbeat that the qualification of the nurses was testimony of Kenya’s high quality and competence of her health workers.

“Today marks a new era of international recruitment of Kenyan nurses to the UK through a government-to-government arrangement. This is a milestone in the implementation of the health workforce collaboration agreement,” said Marriott.

She noted that another 70 nurses have been identified and currently being matched with potential employers in the UK and will be travelling to the UK in the next months.

Further, she urged the nurses to show high standards during their stay in the UK.

Two nurses each were picked from Vihiga and Kirinyaga while one each was picked from Busia, Kisii, Murang’a, Marsabit, Nakuru, Uasin Gishu, Nyamira, Migori and Siaya counties.

Nine Months After Approval, NCLEX Centre in Turkey Yet to Take Off

The joy that enveloped NCLEX test takers in October last year when Pearson Test Centre in Turkey was approved for take off has been shortlived as the Centre is yet to conduct any NCLEX Exam this year typesofnursing.com has learnt.

Our correspondent who has been monitoring the centre observed that exam dates have not been available for test applicants. Nurses from mostly Nigeria and United Arab Emirates have all taken to different social media platform to express their dissatisfaction with Pearson for making the announcement of a new centre when it is not ready to follow it through.

A check this centre this morning by typesofnursing.com shows that there is no available test dates for the rest of this year an indication that the centre might not open for NCLEX Test takers this year.
